The ambulance is a bicycle. In Afuá, a delta town of roughly 38,000 people at the northwestern corner of Marajó, a medical call brings the bicilância — a stretcher frame welded onto a bike, a driver standing hard on the pedals, planks drumming underneath. No siren is necessary, because there is no traffic to part. Motor vehicles have been shut out of this town since 2002, first by custom and since 2022 by municipal law. Afuá sits where the Cajari River spills into Vieira Grande Bay, in the maze of channels the Amazon breaks into before it reaches the Atlantic, and almost none of the town rests on solid ground.
